Transition and School to Work

Wise collaborates with a variety of partners to improve opportunities for students with disabilities as they plan for life beyond high school. Our consulting services include coordination of transition and school related events, facilitation of strategic planning and community meetings, Person Centered Planning, and systems navigation for individuals, families, educators, and community resource providers. Services are customized to meet the unique needs of diverse communities throughout Washington State and continue to evolve as services and systems change.

Our Services

Wise provides information, resources, and support to government entities, community resource providers, educators, students, and families to create successful pathways from school to work and adult life. As the needs of transition services continue to evolve, we work collaboratively with partners to build tools, resources, and opportunities that improve outcomes for students moving into adulthood.
